Technology Device Agreement


District Parent/Student Technology Device Agreement

Your child has been loaned a Schuylkill Haven Area School District technology device for educational use.  It is our belief that if reasonable precautions and care are taken in the use of the device, it should not experience physical damage. Each student and parent/guardian are asked to read this form carefully.

Parents/Students will NOT be given a technology device until a signed form has been received.

  • I/We understand the device and its accessory equipment (technology device and charger) are the property of the Schuylkill Haven Area School District.
  • I/We understand the student, with the support of the parent, is responsible for the daily care and maintenance of the device (this includes various accessories such as the technology device and charger).
  • I/We understand and agree to abide by the rules and regulations of the Acceptable Use Policy (can be found on the SHASD website). Failure to abide by these policies may result in a loss of access as well as other disciplinary or legal action.
  • I/We understand that all device damages or the theft/loss of the device must be reported to building administration within 24 hours of an incident.If not, you will be responsible for the cost of the replacement of the technology device.
  • I/We understand when transported the technology device to and from school reasonable precaution against damage, theft, or loss will be taken.
  • I/We understand if the student did not exercise proper care and/or take appropriate precautions, and this behavior resulted in damage to the technology device, the cost of the repair will be the responsibility of the student/parent.The cost for repairs and replacements is listed on the district website.
  • I/We understand the charging cable and adapter block are not included with the technology fee. If you lose or misplace one or both, you will be required to purchase a replacement from the school district at a cost of $39.99.
  • I/We understand the technology device and its accessory equipment must be returned at the end of the school year or if I no longer attend Schuylkill Haven Area School District. If these items are not returned, I/We are responsible for the cost to replace them. Failure to return the items within 48 hours of the designated end of year date may result in legal action. NOTE: I/We further understand the original district-issued charging cable and adapter block must be returned and I/We will be responsible to pay the replacement cost if the original is not returned.
  • I/We understand inappropriate use of the technology device will result in the loss of privileges, including losing access to the device.
  • I/We understand video or audio recordings of anyone without his/her consent is prohibited and will result in the loss of privileges, including losing access to the device.
  • I/We will not download, install, or use file-sharing programs to access music, video, games, applications, etc. Sending mass email messages to students is not permitted, unless directed by a teacher or administrator.

 

I/We have read and agree to all of the guidelines contained within this document and the current SHASD Acceptable Use Policy #815.
